Fewer health disparities in the Netherlands
GelijkGezond wants to increase the number of healthy life years for people with a low income. Because something isn't right in the Netherlands: the 20% of people with the lowest incomes live on average 9+ years less than the 20% of people with the highest incomes. This program strives for health equality; increasing the number of healthy life years possible for those populations with a lower life expectancy.
Population Health Program
Various forms of support have been developed for many social problems, often through pilot projects. Nevertheless, these solutions are not often structurally linked to the people who would benefit from them.
GelijkGezond wants to move forward by building on these initiatives and making them sustainable and widely available to the people who need them most.
GelijkGezond facilitates professionals with the extra time, financial support and organizational power they need to be able to provide proactive, integral and person-centered support to vulnerable citizens with attention for underlying social problems. For example, helping people find (volunteer) work, reduce debt or arrange housing.
The goal is to demonstrate in four Dutch neighborhoods that these steps improve vulnerable residents' quality of life and reduce reliance on the medical system.
